Patient Forms
Upon entering our office, Jes or Cindy, our receptionists, will welcome you as a member of our family. This paperwork provides us with your health history and information on your condition. New Patient forms can be printed and filled out before your visit, and can be either faxed to our office or brought in with you on your first visit. Click here for New Patient Forms.

Consultation
Next, you will have a consultation with either Dr. Green or Dr. Starr to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
Examination
After your consultation, the doctor will perform a complete orthopedic and neurological examination. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent.
X-Ray Studies
Depending
on your specific condition, we may refer you to have X-rays or MRI
taken. All of our diagnostic studies are taken by Clinical
Radiologists just minutes from the office. All studies are reviewed by a Board Certified Medical Radiologist. We typically can have you
seen the same day, and since we're 'hardwired' with the Radiologists,
you'll never have to bring copies of your films; we can view them live
time on our computers at the same time the Radiologist sees them.
Report of Findings
Once
the information is collected and examinations are performed, the
doctor will give you a detailed report of all findings and answer any
questions including:
- Doctor, can you help me?
- How often do I need to be seen?
- What will my treatment cost?
After
reviewing your health history, goals, and examination, and any needed
diagnostic films, the doctor will discuss with you recommendations and
let you know if your condition requires care with other providers. If
the doctor accepts your case for care, he will review with you the
recommended treatment plan in detail with you.
Treatment
The
doctor will review the different types of treatment that would best
treat your condition/injury. Treatments may include; manual medicine
(manipulation), physical therapy techniques, spinal disc decompression
and/or soft tissue massage and other techniques.
Maintaining Your Health
Prior to releasing you from care, the doctor will suggest a wellness program to incorporate outside of treatment. Advise may include certain activities or positions to avoid, and at home exercises and/or stretches and general healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized plan.
